My documents are in different names. What should I do?

To meet the minimum ID requirements for a police check, all four of the documents you provide must have the same name. If this is not the case and one of your ID documents list a different name to your other ID documents, for example, a different surname, then you must upload a additional document from the list below to support your change of name:

• Australian Marriage Certificate (Issued by state Registry of Births, Deaths and Marriages)
• International Marriage Certificate
• Change of Name Certificate (Issued by state Registry of Births, Deaths and Marriages
• Australian Birth Certificate (With name change listed)

You will be able to upload a change of name certificate or marriage certificate once you have uploaded at least one commencement document, one primary document, two secondary ID documents, selfie ID photo and have moved to the next stage. An example of what you will see is provided below.
