My documents are in different names. What should I do?

To meet the ID requirements for an NCCHC check, all four ID documents must show the same name or a change of name document must be provided. If one document shows a different name, such as a maiden or previous name, you'll need to provide supporting evidence in the form of the following:

  • Australian Marriage Certificate (Issued by state Registry of Births, Deaths and Marriages)
  • International Marriage Certificate
  • Change of Name Certificate (Issued by state Registry of Births, Deaths and Marriages
  • Australian Birth Certificate (With name change listed)

Note: A Celebrant issued marriage certificate, Divorce certificate/order, Statutory declaration or International change of name certificate do not meet the government's ID requirements and will not be accepted as a proof of name change.
