Do I have to disclose my previous name?
Yes, you must disclose all names you have been known by when completing your application.
There are some circumstances where approval may be given to have previous names removed from the final certificate.
These circumstances may include, but are not limited to:
- where an applicant has changed their gender; or
- in instances of domestic violence.
